This is a 3-part carbonless continuous-form security envelope, custom-produced for the brand Bank. It is a tractor-feed, self-sealing document designed to issue, deliver, and record account numbers and password/voucher credentials (PIN mailers) in a confidential, tamper-evident format.

Key Features

Account and voucher grid layout — An Account No. field sits at the top, followed by two columns of Voucher No. entries, letting a single form carry multiple credential records in one pass.

Triplicate carbonless construction — One print or write-through impression transfers to all three plies, producing an original plus two copies for the branch archive, the customer, and the delivery/logistics record — no carbon film required.

Anti-forgery guilloche / mesh pattern — Every voucher-number box is printed over a fine repeating security background that deters photocopying, scanning, and alteration.

Continuous-feed (tractor) format — Sprocket holes run along both edges for automatic dot-matrix/line-printer loading in bulk bank operations.

Perforated tear-open design — Vertical and horizontal perforated lines with directional arrows guide clean opening; the printed instructions tell staff to tear the longitudinal lines first, then the lateral ones, and to leave the dashed strip intact until the form is loaded.

Built-in confidentiality envelope — The form folds and seals into its own mailing pocket, keeping the password hidden until the recipient opens it.

Typical Application

Used by bank branches and call-centre operations to mail bank-card / online-banking passwords and one-time voucher credentials to customers. The three plies create an auditable trail — issuing branch, delivery carrier, and customer copy — while the sealed, pattern-printed body protects the PIN from casual exposure during transit.
