Three page layout: Collector copy, Donor copy and Laboratory copy. No carbon paper is required; information written on the top sheet automatically transfers to the underlying copies for simultaneous record-keeping.

Pre-printed sections guide users through 5 defined stages: Donor information declaration, medication disclosure, collector validation, lab receiving confirmation and MRO result review, covering every link of specimen handling.

3. Unique specimen ID barcode system

Matching, pre-printed barcode stickers with identical serial numbers are supplied. Labels can be peeled off and affixed onto sample containers to guarantee one-to-one matching between paperwork and physical samples and eliminate mix-ups. After removing labels, the main form stays fully intact for lab filing.

4. Tamper-evident security seals
